FIRST TIME PATIENTS, START HERE:
Step 1: Please fill out the sign-up/medical history form given to you by the receptionist. By filling out this form will you help us understand why you are here today and what we can do to serve you. There is a big difference between coming to the dentist for whitening or because your in pain.

Step 2: Once you have filled out the sign-up/medical history form, the dentist will direct you to information within this guide, and attempt to help you understand the dental problems your facing.

Step 3: Now that you understand what the problem is the dentist will work together with you in quoting you a general number of required appointments to finish your treatment and total cost.
